Useful assessment identifies initiating event, exposure, escalation, people and assets at risk, existing barriers, consequences and the conditions that could make controls fail.

Separate prevention, detection and mitigation

Process controls, inspection, alarms, shutdown, passive fire protection, firefighting, emergency response and recovery each play different roles and need an accountable owner.

Close actions through verification

Every recommendation should have priority, owner, due date, acceptance criteria and evidence of completion. A closed spreadsheet cell is not proof that field risk changed.

Project-specific verification

Codes, approvals, credentials, system evidence and authority requirements change. Confirm the current legal entity, scope, project specification and acceptance route before procurement or installation.
